What is PCNL Surgery?
Percutaneous Nephrolithotomy (PCNL) is a minimally invasive surgical procedure used to remove kidney stones.
This advanced technique allows urologists to access and remove kidney stones through a small incision in the patient’s back,
PCNL is highly effective for treating stones that are too large to pass naturally or are resistant to other treatment methods.
When is PCNL Surgery Required ?
PCNL surgery is surgery of choice in following cases:
- Large Kidney Stones: When kidney stones are larger than 10 mm or have failed to pass through natural means, PCNL is often the preferred method for their removal.
- Complex Kidney Stones: PCNL is suitable for treating stones that are irregularly shaped or have become lodged in the kidney.
- Kidney Stones that cause Blockage or Infection: When kidney stones cause blockage in the urinary system or lead to recurrent infections.
- Ineffective Non-invasive Treatments: If other non-invasive treatments such as shock wave lithotripsy (SWL) or ureteroscopy have failed to resolve the kidney stone issue, PCNL may be the next step.
The PCNL Surgery Procedure
During a PCNL procedure, the patient is Usually placed under spinal Anesthesia.
The surgery is performed by a Qualified urologist and operation Theatre Staff.
- Preparation: The patient is positioned on their stomach to access the kidney through the back. The surgical team then uses imaging machines (C-Arm Intensifier) to precisely locate the kidney stone.
- Incision: A small incision, usually around 1 cm, is made in the patient’s back to access the kidney. This allows the urologist to insert a hollow needle into the kidney while guided by the imaging system.
- Guidewire Placement: A thin guidewire is carefully inserted through the needle and into the kidney to create a pathway for the PCNL instruments.
- Dilation and Access Sheath Placement: The access sheath, a tubular device, is gently placed over the guidewire to dilate the tract, allowing the urologist to access the kidney stone.
- Stone Removal: A nephroscope (a thin, flexible instrument with a camera) is introduced through the access sheath. The urologist visualizes the stone on a monitor and uses specialized tools to break it into smaller fragments. These smaller pieces are then removed through the nephroscope .
- DJ Stent Placement :A ureteral stent (pipe) is usually placed to help the kidney drain urine and aid in the healing process.
- Closure: Once the stone is entirely removed, the access site is closed with stitches or covered with a sterile dressing.
Advantages of PCNL Surgery
- High Success Rate: PCNL is highly effective for removing large and complex kidney stones, achieving a high success rate in stone clearance.
- Minimally Invasive: Compared to traditional open surgery, PCNL is minimally invasive, leading to less postoperative pain and a shorter recovery time.
- Single Treatment Session: In most cases, PCNL allows for the complete removal of the kidney stone in a single treatment session, However large stones may require multiple sittings.
- Reduced Risk of Complications: PCNL carries a lower risk of infection and other complications compared to other surgical options for large kidney stones.
Recovery and Aftercare
Following PCNL surgery,
Patients usually need to stay in the hospital for a day or two for monitoring and pain management.
The urologist may advise a restricted diet and increased fluid intake to facilitate healing and prevent stone recurrence.
Most patients can resume light activities within a week but should avoid strenuous physical activity for several weeks.
Regular follow-up visits with the urologist are essential to monitor the healing process and address any concerns.
